Below are downloadable copies of the paperwork we need for your student to Travel with GWSD.
If your student traveled with the team in 2024-25
YOU DO NOT NEED TO FILL THESE FORMS OUT AGAIN.
The documents can be returned to Miles Owens as hard copies, or scaned and emailed to [email protected].
Please note, you will need to have one of the documents notarized You can make an apointment at the GW front office to have this done. We will also have notaries availble at the parent meetings we hold before travel tournaments with large student participation. For trips with smaller student participation we use email to communitate important information.
